Electronic Medical Forms
Your group will receive a link to your electronic medical form upon completion of your Scheduling Form. If you need the electronic version sooner please email our Scheduling Manager.

This will be a Google Form, and easily sent to your student families and adult chaperones via a link. It is:
- Completed using any devices (computer, phone, iPad, etc.) using WiFi or cellular.
- Automatically translated into Spanish and Somali. Translated via browser into any other language.
- Data accessible to the teacher
The teacher will go through the compiled information to ensure everyone has completed it, and enter in the student and chaperone letter group and dorm room assignments.
All forms must be completed by the MONDAY OF THE WEEK PRIOR TO your trip (appr 7-10 days before your trip). Our staff will access the data and adequately prepare for your visit since all of the needs (dietary, medical, etc) are aggregated in this form.

- Chaperone Background Checks (as required by school)
- Bussing/Transportation

6 Months Before Your Trip

- Reservation Contract (to be provided by our Scheduling Manager)
3 Months Before Your Trip
2-4 Weeks Before Your Trip
Review:
- Evening Activity Lesson Plans
- Proposed Schedule (to be provided by our Scheduling Manager; email any changes or questions within the week)
Optional:
- Preview Dorm Room Layout
- Review and lead Pre-Trip Classroom Activities
- Review and print Trip Journal Pages
- Assign students and chaperones to KP Duty
- Assign chaperones to off time or dorm duty rotations
Distribute to students and parents:

Medical Release & Liability Forms:
- Ensure every chaperone, teacher, & student has completed a form
- Assign letter groups in the Reponses sheet
- Assign dorm rooms in the Responses sheet (many teachers use CleverGroups)
- If accommodations are needed for accessibility, email the Outdoor School Manager
1 Week Before Your Trip
Finalize details:
- Review Code of Conduct with students
- Review/set Dorm Rules with students
- Distribute Packing List to students and chaperones
- Trip Schedule and any other reminders
- Pre-trip Classroom Activities
Confirm with our Scheduling Manager:
- Final numbers of students and adults
- Arrival and departure time
During or After Your Trip
Optional:
- Each teacher who attended the trip may complete Continuing Education Unit Approval Form
- Lead Post-Trip Classroom Activities
- Debrief and/or collect trip journals
